Total testosterone
This measures testosterone circulating in the blood in both bound and unbound forms. Because levels vary, an isolated low result is usually confirmed with another early-morning test before a diagnosis is made.
Free testosterone and SHBG
Most testosterone is attached to proteins. Free testosterone is the unbound portion. Sex hormone-binding globulin, or SHBG, affects the relationship between total and free testosterone. A provider may consider these results when total testosterone and symptoms do not tell the same story.
LH and FSH
Luteinizing hormone and follicle-stimulating hormone are signals from the pituitary gland. Their pattern can help distinguish a testicular problem from a pituitary or hypothalamic problem and guide additional evaluation.
Complete blood count
Hemoglobin and hematocrit show how concentrated your red blood cells are. Testosterone can raise these values, which is why a baseline and ongoing monitoring may be important. A high result deserves clinical review—not an automatic dose increase or an improvised fix.
PSA and prostate assessment
Prostate-specific antigen may be considered before and during treatment based on age, symptoms, risk, and shared decision-making. An abnormal result does not diagnose cancer by itself, but it may require additional evaluation.
Estradiol
Men naturally produce estradiol, partly by converting testosterone. The number should not be treated in isolation. Symptoms, assay method, testosterone dose, and the full clinical picture matter, and lowering estradiol is not automatically beneficial.
The useful question
Instead of asking whether every value is “optimal,” ask what the pattern means, whether it is repeatable, what might be causing it, and whether treatment benefits are likely to outweigh the risks.
