Injection therapy is effective, but it's a real medical treatment with real risks. We'd rather you know them up front than find out later.
Priapism (erection lasting over 4 hours)
The most serious risk. A prolonged erection is a medical emergency — go to the ER immediately, as untreated priapism can cause permanent damage. Correct dosing, which is why we start in office, makes this uncommon.
Pain or aching at the injection site
Some men feel a dull ache, often from the alprostadil component. It usually eases as your body adjusts, and the formula can be adjusted if needed.
Bruising or minor bleeding
Small bruises can happen, especially early on. Proper technique — which we train — keeps this minimal.
Scarring with long-term use (fibrosis)
Repeated injections can, over time, cause scar tissue. Rotating injection sites and following your dosing plan reduces the risk. We monitor for it.
Injection therapy may not be appropriate for men with certain blood disorders (such as sickle cell disease), a history of priapism, certain penile scarring conditions, or a penile implant. Your provider will review your history first.
Medical emergency: an erection lasting more than 4 hours requires immediate care — go to the nearest emergency room. Untreated, it can cause permanent damage.

Do I have to come in, or can this be done discreetly?
Both. You can be seen in person in Beulaville, or handle the entire thing by telehealth as an NC resident — evaluation by video, prescription sent to your pharmacy. Most men prefer the privacy of telehealth for this one, and that is completely fine.
Is ED ever a sign of something bigger?
Sometimes, yes. ED can be an early signal of cardiovascular issues, low testosterone, or other conditions — which is why a provider evaluates you rather than just handing over pills. If we spot something worth looking into, we will tell you.
Can I combine ED treatment with TRT?
Often they go together. Low testosterone can contribute to ED, and treating it can help — though not always on its own. If both are in play, we treat the whole picture instead of one symptom.
Why does injection therapy require an in-person visit when the pills don’t?
Oral medications (sildenafil, tadalafil) are low-risk and available by telehealth statewide. Injection therapy is different: getting the dose right matters a great deal, because too high a dose can cause a prolonged erection (priapism). For your safety, we give and observe your first test dose in our Beulaville office, dial in your dose, and train you to self-inject. After that, refills are handled by telehealth. This isn’t red tape — it’s the standard of care done properly.
How much does injection therapy cost?
A 5 mL bottle is $175, which is generally good for about 10 injections at max dose — and more if your effective dose is lower. Supplies and your in-office training are included. Because the dose is customized, your provider will confirm what your bottle will realistically last at your visit.
